Mission Statement
Heartsound LLC is a DDD/Medicaid approved provider in the state of New Jersey. We provide community-based services for adults with intellectual and developmental disabilities. We believe that everyone deserves to fully participate in their community, both vocationally and recreationally to the extent that they choose.

Community Inclusion Services
Community Inclusion Services
Services provided to a participant either in-person or virtually to support and assist participants in educational, enrichment, or recreational activities.
Community Based Supports
Community Based Supports
Service to assist participant to fully engage with the community. Services can take place either inside or outside of the home. Only available to those on SP Waiver.
Individual Supports
Individual Supports
Service to assist participant to fully engage with the community. Services can take place either inside or outside of the home. Only available to those on CCP Waiver.
Natural Supports Training
Natural Supports Training
Training and counseling services for individuals who provide unpaid support, training, companionship or supervision to participants. For purposes of this service, individual is defined as: “any person, family member, neighbor, friend, companion, or co-worker who provides uncompensated care, training, guidance, companionship or support to a participant.”
Introduction to the Arts
Introduction to the Arts
Every quarter, HeartSound will present different creative demonstrations to reinforce the skills that our clients have learned.
Prevocational Training
Prevocational Training
Services that provide learning and work experiences, including volunteer work, where the individual can develop general, non-job-task-specific strengths and skills that contribute to employability in paid employment in integrated community settings.
Supported Employment
Supported Employment
Activities needed to help a participant obtain and maintain an individual job in competitive or customized employment, or self-employment, in an integrated work setting in the general workforce for which an individual is compensated at or above the minimum wage, but not less than the customary wage and level of benefits paid by the employer for the same or similar work performed by individuals without disabilities.
Respite
Respite
Services provided to participants who are unable to care for themselves on a short-term basis because of the absence or need for relief of those persons who normally provide care.
